You may find the documents required when reporting damage listed below. You are required to report the damage as soon as possible from the moment it occurs.
For Medical Indemnity Insurance:
- Request letter of the injured person from the insured
- Written statement of the insured
- Epicrisis report (of the doctor / from other doctors, if they intervened/ the hospital, each provided separately)
- Surgery notes
- Patient information / consent form
- Electronic records
- Consultation report
- Correspondence records with the court, ministry and inspection board
- Copy of invoice issued to the patient
- Laboratory results
- Dispatch document
- Hospital discharge certificate
- Certificate of specialization, diploma of the insured doctor
Apart from the list of required documents above, different documents may be requested depending on the characteristics of each event.
For Freight Insurance:
- Request Letter,
- Original consignment note (if available),
- Original Receipt,
- Original customs certificate,
- Original yükleme-loading itinerary (if available),
- T.İ.B 1 form (if available),
- Original Insurance policy,
- protest against the carrier.
Apart from the required documents listed above, additional documents may be requested depending on the characteristics of each case.
For Breach of Trust Insurance:
- Statement of the insured,
- Claims Letter stating damage,
- Insurance Policy,
- Police report,
- Passport/National ID details of the insured,
- Recorded camera footage (if available).
(To request a damage claim under Breach of Trust Insurance, the person committing the breach of trust must be a contracted employee in your company/workplace. Otherwise, your damage claim will not be valid.)

For Employers’ Liability Insurance:
- Claims letter,
- Payroll sheet or Social Security registration,
- workplace accident report,
- Passport/National ID details of the injured parties,
- Statement of the insured,
- Insurance Policy.
In case of death:
- Death Certificate,
- Proof of income,
- Certificate of Succession,
- National ID details of the successor.
In case of request for treatment:
- Report of incapacity to work,
- Original receipts/prescriptions.

For Professional Liability Insurance:
- Accident report stating damage,
- Death/Injury inspection report,
- Certificate of Succession,
- National ID details of Successor,
- Passport/National ID,
- Payslip,
- Statement of the Insured,
- Insurance Policy.

For Personal Liability Insurance:
- Accident report stating damage,
- Expense documents for treatment costs,
- Insurance Policy,
- National ID details of person causing injury
- National ID details of injured party
- Claims statement for damage cost.

For Elevator Liability Insurance:
- Official accident report,
- Claims request by injured party to the insured who has caused injury,
- Proof relating to the damage incurred (surveyor report, expert report, etc.)
- Maintenance contract of the elevator,
- Statements of incurred maintenance,
- Signed original documents by the injured person (In case of death, a certificate of succession is required. If the injured person is under the age of 18, their guardians/parents must sign in their place.)
- Insurance policy,
- Identity card details.
